辅助生殖医学科Assisted Reproductive Medicine

辅助生殖医学科于20世纪80年代起，在已故国内著名妇科内分泌专家邵敬於院长带领下，在不孕症治疗方面率先运用国产药物联合诱发排卵。90年代起开始体外受精-胚胎移植临床应用的研究，1998年诞生我院首例 “试管婴儿” 。1999年，一名绝经3年的57岁妇女在我院经赠卵“试管”受孕分娩。2002年医院通过原上海市卫生局审批开展夫精人工授精（AIH）技术。2006年通过原国家卫生部审批，体外授精胚胎移植（IVF-ET）/单精子卵胞浆内注射技术（ICSI）。2010年通过原上海市卫生局审批开展供精人工授精（AID）。2018年通过上海市卫健委审批开展植入前胚胎遗传学诊断（PGD）技术。目前，医院获准开展所有辅助生殖技术准入项目。是医院重点科室，2019年度获上海市卫生系统先进集体称号。现有医护人员72名，包括中华医学会生殖医学分会常委1人，国家重点专项首席（青年）科学家1人，国家自然基金优青1人；共有主任医（技）师9名、副主任医（技）9名、研究员1名、副研究员1名，博士研究生导师3名，硕士研究生导师9名。临床医技人员基本上具有硕士以上学历，其中博士31名。开设了男性科、生殖遗传咨询、中医助孕、不孕不育、反复种植失败、PCOS助孕、生育力保存、生殖内分泌等专病门诊。2021年完成取卵周期7341个，门诊量20多万。

近5年，发表论文61篇，其中SCI论著17篇，获局级以上课题18项，科研经费1162.7万元：国家自然科学基金项目6项，优秀青年医学人才培养计划1项，启明星计划1项，发明专利2项，实用新型专利2项。牵头制定了中华医学会《ICSI的操作指南》。围绕不孕不育与配子及早期胚胎发育异常这一问题，获得了多项国家及省部级课题支持。2016年首次报道了哺乳动物早期胚胎发育过程中组蛋白修饰建立过程；并利用微量组学技术揭示了克隆胚胎发育阻滞的关键分子机制；揭示了透明带异常导致不孕的机制并提高了短时受精胚胎的发育。相关成果发表在Nature、Cell Stem Cell、Human Genetics及Fertility and Sterility等杂志。研究成果曾入选中国科协2016年度“中国生命科学领域十大进展”。男科实验室的精液分析获德国男科学会质控证书。科室为患者提供个性化的治疗方案和更贴心、暖心、舒心的服务，帮助患者早日圆梦。

 Assisted Reproductive Medicine Department

The Assisted Reproductive Medicine department has existed since the 1980's. Under the leadership of the late renowned domestic gynecological endocrinology expert, Dean Jingyu Shao, we were the first to use domestically produced drugs in conjunction with induced ovulation in the treatment of infertility. Since the 1990s, we started research on the clinical application of in vitro fertilization and embryo transfer. In 1998, the first "test-tube baby" was born in our hospital. In 1999, a 57-year-old woman who had been menopausal for three years was successfully impregnated through egg donation and "test-tube" conception. In 2002, our hospital was authorized by the former Shanghai Municipal Health Bureau to implement Artificial Insemination by Husband (AIH) technology. In 2006, approval was granted by the former National Health Ministry for In-Vitro Fertilization Embryo Transfer (IVF-ET) / Intracytoplasmic Sperm Injection (ICSI). In 2010, the former Shanghai Municipal Health Bureau allowed us to perform Artificial Insemination by Donor (AID). In 2018, approval was garnered from Shanghai's Health Commission to implement Preimplantation Genetic Diagnosis (PGD). Currently, the hospital is authorized to carry out all the assisted reproductive technology project. It’s a key department in the hospital and was awarded the title of Advanced Team in Shanghai's health system in 2019.

The department has a total of 72 medical personnel, including one standing committee member of the Reproductive Medicine Branch of the Chinese Medical Association, one chief scientist for the national key project (youth), and one key project of the National Nature Fund Young Scientist; 9 chief doctors (technician), 9 deputy chief doctors (technician), 1 researcher, 1 deputy researcher, 3 doctorate tutors, and 9 master tutors. The clinical medical technicians have a master's degree or above, among which there are 31 doctors. We have opened several specialty clinics including men's department, reproductive genetic counseling, traditional Chinese medicine assisted conception, infertility, recurrent plantation failure, PCOS conception, fertility preservation, and reproductive endocrinology. There were 7341 egg collection cycles and 200,000 outpatient visits in 2021.

Over the past five years, 61 papers have been published, including 17 by SCI; 18 projects at the bureau level and 11.627 million yuan in research funds were obtained. These include the National Natural Science Foundation Project 6, the Training Program for Excellent Young Medical Talents 1, the Qiming Star Project 1, 2 invention patents, and 2 utility model patents. The department leads the establishment of the Chinese Medical Association Guidelines for ICSI. 

It has received the support of multiple national and provincial/ministerial-level projects centered on the problem of infertility and abnormalities in gamete and early embryonic development., such as the first report on the establishment of histone modification in the early embryonic development process of mammals in 2016. Using trace-omics technology, it revealed the key molecular mechanism of cloning embryonic development stagnation; it revealed the mechanism of zona pellucida abnormality leading to infertility and improved the development of short-time fertilized embryos. The related achievements are published on Nature, Cell Stem Cell, Human Genetics, and Fertility and Sterility magazines. The research results were selected as one of the "Top Ten Advances in China's Life Sciences in 2016" by the China Association for Science and Technology. The semen analysis of the menses laboratory has been certified by the German Men's College.

The department provides patients with personalized treatment plans and more intimate, warm, and comfortable services, aiming to help patients fulfill their dreams at an early date.

科室概况

上海市第一妇婴保健院辅助生殖医学科于2006年通过国家卫生部审批，是上海市最早开展辅助生殖技术的机构之一，目前开展体外受精-胚胎移植（IVF-ET）、卵胞浆内单精子注射(ICSI)、植入前遗传学诊断（PGT）、供精人工授精(AID)、夫精人工授精(AIH)等项目。年平均门诊量近15万人次，年完成体外受精- 胚胎移植（试管婴儿）取卵周期6000余，完成胚胎移植近7000周期，移植周期临床妊娠率50%，人工授精1000 余周期。

辅助生殖医学科目前分为三个亚专科，其中生殖一科依托于一妇婴“善良医院”的医院文化，秉承“温暖生殖一科”的理念，注重患者就医感受，竭诚为广大不孕症夫妇解除烦恼，圆你们全家团圆的梦想。生殖一科现有主任医师两名，副主任医师两名，主治医师三名，住院医师四名，其中博士研究生导师1名，硕士研究生导师1名，博士后一名，完善的团队合作保证了患者的就医质量。

如同所有的医学一样，我们无法确保每个患者每次都能成功怀孕，但我们会根据患者的具体情况，制定最合适的治疗计划。我们重视患者的心理感受，注重与患者的沟通，安全、舒适的妊娠一直是我们追求的目标。

秉承个体化治疗理念，实现精准助孕

2015年美国时任总统奥巴马在国情咨文中提出“精准医疗计划”，一时间“精准医疗(Precision Medicine)”成为覆盖全球的热门话题。目前，在医学各个领域迅速发展。生殖一科作为一个经验丰富而质控严格的科室，在坚持辅助生殖技术促排卵方案标准化的基础上，根据每个患者的特点，为患者制定个体化的促排卵方案和药物剂量，实现精准助孕。我们根据患者的年龄、卵巢储备、体重、卵巢手术史、既往促排卵结果等指标，设计个体化的用药，以期获得适量的目标卵子数，为实验室提供高质量的卵母细胞。

除此之外，科室还开设男性不育症的诊断与治疗，从男性因素探寻不孕不育原因。增添生殖遗传门诊、多囊卵巢助孕门诊以及为反复种植失败、复发性流产开设专科门诊。根据患者情况、不孕原因等综合评估，制定不同的辅助生殖治疗方案，做到“因人而异，量体裁衣”的个体化治疗。

立足科研，造福患者家庭

生殖一科注重学术交流及推广，为解决人类辅助生殖技术中的难点及热点问提，多次成功举办“二孩政策下高龄女性助孕策略”学习班，获得了与会专家及学者的一致好评。同时，科室鼓励每一位医生积极投身生殖医学的基础研究，从微观的角度发现更多的致病机制并探索治疗手段。在科室领导和每一位员工的努力下，近年来科室共获得批准课题多项，总项目经费近200万元。其中国家自然科学基金委项目5项，上海市科委课题3项，上海市申康三年行动计划1项，拥有发明专利2项。

寻找不明原因不孕不育的遗传病因一直是个世界难题，刘文强博士和科研团队成员们发现这些患者中有很多与配子及早期胚胎发育异常有关，通过全基因组外显子测序技术确诊了多例不明原因不孕患者的病因。我们注重在临床上发现问题，并将其上升到科研高度，试图去探究临床问题背后的原因与解决办法。我们发现一名透明带缺失的患者，通过家族测序，发现这是一例由ZP2、ZP3双杂合突变导致女性不育的病例，通过动物及细胞模型阐明了其致病机制，患者经过进一步治疗，最终成功受孕并产下健康男婴，这一成果发表在《Human Genetics》杂志上。正因为胚胎实验室的科学家们对临床问题的不断探索，攻坚克难，从科研角度发现了病因，并着结合临床真正造福于一部分存在遗传问题的疑难不孕患者家庭。

人文关怀，贴心管理

“有时能治愈,常常在帮助,总是在安慰”，这句著名的特鲁多医生的座右铭，是生殖一科主任李昆明主任经常用来勉励科室年轻医生，并且身体力行的工作准则。

李昆明主任不仅关注患者的病情，更加关注患者的内心。因此，在繁忙的工作之余，李主任开通了“李昆明医生”公众号，对患者的服务延伸到八小时以外。公众号的留言和互动非常踊跃，连续三年年（2018-2020年）入选上海市十大健康微信公众号综合指数排名（科室/个人类）前十。此外，李主任2018年原创了一首歌曲《生命的曙光》于公众号上发布，给了那些在试管道路上前行的夫妻们温暖的力量。

在李主任的带领下，科室范宇平医生、杨阳医生等都在工作之余利用自己的时间写科普文章，为更多群众普及生殖健康的知识。同时科室推出了多种宣教途帮助患者对治疗过程更加了解。除了传统的文字资料的发放，另还推出了视频课堂循环在门诊大厅播放，患者也可以自行扫描二维码，在家通过网络收看到视频课堂的所有内容。

另外，在工作流程管理上，生殖一科采用责任制医护团队管理服务模式。优势在于患者从初诊开始的所有环节均由固定的医护团队负责管理，让医生和护士对所负责的患者更有责任感；连续性的就诊记录让医生处理患者的问题更有效、直接、迅速；同时患者也不会因为频繁更换医生而导致对医生缺乏信任，反而增强患者的就诊安全感。

护理团队也在2018年积极探索个案护理管理模式在辅助生殖医学科门诊中的运用，以期在现有的医护团队模式下能够充分发挥护士在心理评估、疏导、个体化宣教等方面的作用。

同时，科室管理采用核心小组成员管理模式，团队成员由科室主任、临床负责人、实验室负责人、护士长及临床各组组长组成。每周定期召开核心小组成员会议对近期科室管理中存在的问题进行讨论及方案落实。这些制度较大程度上缩短患者的就医等待时间，把更多的时间还给了患者，为患者提供更好更优质的服务。

未来规划

对于未来发展，同济大学附属第一妇婴保健院生殖一科将始终秉持“人文关怀，温暖一科”的理念。通过细心呵护，不断改进医疗服务方式与流程，让患者缓解焦虑、感受温暖。在技术上我们努力进一步改良，增强治疗效果；继续探索新技术，为疑难杂症的患者提供新的治疗。努力为广大不孕不育患者提供最优质可靠而又温暖贴心的生育诊疗服务。

Associate Chief Physician, Doctor of Medicine, Lecturer at Tongji University Medical School, graduated from Tongji University, School of Medical, once went to the University of Nebraska Medical Center for exchange studies. She has been engaged in assisted reproductive technology for nearly 10 years, specializing in the diagnosis and treatment of infertility, especially assisted reproductive technology for conception (artificial insemination, in vitro fertilization), with profound insights into conception assistance for patients with polycystic ovary syndrome and diminished ovarian reserve function. In recent years, her main research focus has been on the effects of oxidative stress on eggs and embryos, and premature ovarian failure. She has published more than 10 papers in SCI and domestic journals and participated in the translation of foreign obstetrics and gynecology books.
